APPLICATION FOR CONSIDERATION AS A CANDIDATE FOR THE BOARD OF DIRECTORS OR DISTRICT DELEGATE
Under penalty of perjury, I, the undersigned, declare that I meet the requirements to submit my application and statement as a candidate for a position of District Delegate or Board of Directors (outlined on the nomination form mailed to you) for the upcoming annual election. I further agree that the information provided below is true to the best of my knowledge. I am also aware that my submission will be published to my community as submitted, including typos and omissions. (Email and phone will not be published). I will not hold HOA Elections of California responsible for the publishing of my information as I have submitted it. I understand that my statement is not considered received until acknowledged by a confirmation from HOA Elections and that it is my responsibility to follow-up on any submissions if I do not receive a confirmation. This submission may be revoked by the HOA Elections Inspector at any time if it is found to be fraudulent or my qualifications are discovered to be lacking according to my community governing documents.
